Community Health & Education, Inc. is a community health agency located in Chicago. The agency participates in the revitalization of the community. It offers health education and partners with groups, including the Infectious Disease Physician group, to promote healthy communities.

The agency will offer a disease prevention teaching program to address some public health issues the Chicago Lawn community faces.
